Vlan 11 I believe should be untagged if it isn't access ports on all ports connecting to end devices.
I am just getting over the same hurtles but got mine working by changing the settings to reflect what I have stated. DHCP can cause issues quite a bit if not configured consistently within the network for vlans.

Cisco SMB Switch VLAN Configuration
Previous Lesson How to configure Trunks. Next Lesson So I have been at this for about a week now with a lot of trouble along the way. Everything is working so far and we can even register our Cisco IP phones. The problem is, they will only register as an access port assigned to our voice VLAN. If I try to trunk to port to allow both Voice and Data, the phone refuses to register.
The only way I have been able to register the phone is by making the port an access port and allowing the Voice VLAN specifically, doing this no data can transfer to the attached computer. If you don't need the smart port function turn it off.
That "feature" kicked me in the pants for 2 days. I would set a trunk vlan on a port, plug in a computer and the smart port would reassign the port back to vlan 1. If I reset the port it would stay the way I wanted until I unplugged and plugged in the device back in. Now if you want to use the smart port function, it does have its uses.
You just need to set it so that when it sees a phone it sets the right vlans specific for a phone assuming a PC will be behind the phone.
You will want the smart port function to tag the voip lan and untag the data lan. Access ports belong to only one VLAN, trunks belong to all by default. If you do not specify a VLAN for an access port, it defaults to 1. Also, access ports do not tag frames either dot1q or isl which is required for trunks to separate traffic.
Second, you are thinking of traditional ios that has the voice vlan command and makes what is operationally a trunk port seem like a 'special' access port. Then of course you need your voice server on the same VLAN probably on an access port or if you are hosted you may need inter-vlan routing to let them reach the Internet or wherever. Sorry for the delay, just got back into work. We have a couple of different models of Cisco IP phones, the one I am specifically using to make sure they work with the switch is a CP Brandon, I'll try to turn off the macro smartport and see if that helps at all.
We are running the switches in Layer 2 mode and would prefer to keep it that way if at all possible with our setup. Also, thanks!
